I'm looking for a self-hosted forms builder. Something like Google Forms, Zoho Forms, or maybe Formspree.

Any suggestions?

If you already have Nextcloud running you can use the Nextcloud Forms app

It's really basic, I wouldn't recommend it as a form builder, more like a nice added feature.

Baserow and n8n both have form builders built-in, so if you’re using one of these tools already it might be worth checking out.

There is Ohmyform which is great but not updated at this point.

Lime Survey. I needed a self hosted solution and after a long search and evaluating a lot of alternatives, finally settled on this because a lot of advanced features were needed.

Pros: all possible features that you could need and a lot more. Admin interface is very powerful. Easy to install, extend. Cons: PHP. Complicated (as it' has a lot of options). Scripting language is a pain in the butt.
