Is it just me or is the whole Karlach romance EXTREMELY well done? I know I'm a big ol' weird softie and your mileage may vary, but it had me laughing and crying and cheering!

I just couldn't help but feel her frustration, be touched by her love for my fictional avatar and her giddiness the morning after is one of if not THE most touching example of infectious giddy happiness I've ever seen in a game, he'll in any fiction at all!

Hats off to the the team who created, drew and brought to life the visual aspects of the character and to Samantha Béart for a masterful achievement in voice acting!

if that nice moment with her was both seated talking, then that's the first scene. One thing you can do with followers once their approval is high enough, is to go back to act 1 and spam partial long rests to trigger the remaining act 1 scenes. Then go to act 2 and do the same to see if new ones pop up.
