Yeah, OP didn't specify but this is pretty much entirely about the USA's disastrous public health officials and our growing right-wing population.
We also have a growing left-wing population. The US is polarizing and more people are becoming class-aware.

what we know that's on the table
Who is we, and what's on the table currently ?
But let's make a few prediction for end of 2028

First wave of climatic refugee to hit the west. We start to see place with more intense flooding or deadly heat-wave, but not yet at the point people massively seek asylum far-away, but this is getting close

The mess in middle-east won't get better

The conservative wave isn't stuck to Murica, I see a couple of European countries more having either far-right at the goverment or with blocking-minority

Some progress in cancer research, it's easy it's moving every-day

IA industry crash, nowadays, there is tons of cool stuff, but beside AI generated catgirls, and instagram bots, there is no big application, and I don't expect the tech to be mature enough in the coming 5 years, leading to some bankrupcy. It'll be enough to turn mainstream social media in Bot-infested hell

Still on the technology side, I don't expect all these cool start-up about fusion to go somewhere, some would announce major delay (ITER won't start by then, but we know-it) or pivot

China keep rising in power
National institutions are going to fail as Trump reclassifies civil servants as political appointments. Only those loyal to trump will remain.
There will be a substantial brain drain as people with means and motivation leave the US for elsewhere. This will mostly be college educated people who lean center to left of center. There is likely to be an influx of political refugees to New Zealand, western Europe, and the Nordic countries. This means leftists who can't leave will be stuck here. This will push the US further right, and push the other countries further right because the people leaving aren't as far left as they think they are.
Business regulations will be lax, and enforcement will be diminished. OSHAs budget will be cut even further, leaving states to manage workplace safety. Businesses will migrate towards red states where operations are cheaper.
Blue states will start banding together on their own enforcement systems for everything that the federal government used to manage. We already see this to some extent with manufacturers tending to comply with California regulations for nationwide stuff.
